Echocardiography
Echocardiographic evaluation Right parasternal 2D, M-mode, and pulsed wave and color flow Doppler longitudinal and cross sectional images were reviewed. Diastolic dimensions of interventricular septum (IVSd= 6.0 mm) and left ventricular free wall (LVFWd= 4.9 mm) are within reference limits. Diastolic internal diameter of the left ventricle (LVIDd= 27.0 mm) is mildly increased (dilated). Contractility is increased (fractional shortening approximated at 48%). The left atrium is slightly dilated. The leaflets (septal > posterior) of the mitral valve are thick. Mitral insufficiency is noted with pulsed wave Doppler. The right heart is unremarkable. Tricuspid insufficiency is not seen.
Interpretation
Mitral valve endocardiosis (degenerative valve disease) is suspected. Subsequent mitral insufficiency has caused slight left atrial enlargement, mild left ventricular enlargement, and mild increased fractional shortening. Overall, disease is mild and compensated. Therefore, reported coughing and sneezing are likely caused by respiratory disease. Evaluate thoracic radiographs if not yet done. A CBC may also be informative.
Recommendations and Therapy
Additional comments ECG: Rate is fast and rhythm is sinus. Sinus tachycardia may be caused by stress/excitement. Complexes are normal.
Therapeutic considerations
No cardiac medications are currently recommended for this patient with mild disease. An antibiotic may be appropriate if coughing and sneezing are caused by bacterial rhinitis/pneumonia.
